• When completing the request for envelopes, please estimate one envelope per child, and a few spare.
• Detail on the order envelope to complete by parents is:

Child's Name, Year Group, & Quantity

• The order envelopes will arrive at your school before the show date as submitted on this form ready for distribution.
• I will collect the sealed order envelopes from school on the closing date.

• Price per dvd to parents - £5
• Leavers DVDs £5 to parents - £3.50 to school for bulk orders.
• School receives 10% of total order amount which is donated in cash when the orders are delivered.
• DVDs now come in black 14mm cases with a plastic sleeve for paper insert.
